OnLok Lifeways, a Program of All-inclusive Care for the Elderly (PACE), provides long-term care for eligible seniors living in San Francisco and Fremont, CA.The program offers full medical care and support services with the goal to help seniors live at home and in the community for as long as possible.This comprehensive health plan includes medical care, as well as prescription drugs, adult day health services, home care, transportation and other support services.With over 30 years of experience caring for seniors, On Lok SeniorHealth is the model for other PACE organizations throughout the United States. 

Seniors eligible for the SeniorHealth plan are 55 years of age or older, live in San Francisco or Fremont, Calif., and require ongoing medical care and supportive services to continue to live safely at home.
